  1. Enter the desired number of surveys
  2. Enter a proposed Organization Name
    no more than 12 characters.
    Examples: BofA1, IBM1, VDOT1, DeptGenSvcs1, etc
    (note, we suggest adding a number at the end to distinguish versions)
  3. Enter a proposed Admin Password
    letters and numbers only, please
    (be creative - you don't want unauthorized users modifying your survey)
  4. Fill in other information
