  When you click "Submit" you will be taken to a page that will take your credit card information (VIsa or MasterCard). Then, you will receive notification that your account is established. You'll get a url, and confirmation of your id and password. You will then have Administrator access.

  As an Administrator, you may download instructions for preparing your survey, as well as survey templates which can be used "as-is" or modified. The instructions will describe how to create, upload, test, and activate your survey.
Account Name Use letters and numbers, no spaces or punctuation, no more than 12 characters. This is usually a form of your company or organization name.
Examples: BofAmgrs, HarvardLaw, CalDeptFin, Acme03
Administrator Password Please do not choose something obvious. Avoid variations on your account name or "PulseCheck." This password is used to access and modify your account. Do not give this password out to others.
Participant Password You will announce this password to those you want to have take the survey. This one should be easy for the survey population to remember. Participants will need this password and the account name to take the survey.
