Creating an account is fast and easy

  1. Enter the desired number of surveys.
  2. Click on "x $34.95" to verify the total.
  3. Fill out the rest of the form.

  When you click "Submit" you will be taken to a page that will take your credit card information (VIsa or MasterCard). You will receive notification that your account is established. You'll get a url and confirmation of your id and password. You will then have Administrator access to SBPT.
  x $34.95 =  Enter the desired number of surveys. Then click "x $34.95" to see the total.
Account Name Use letters and numbers, no spaces or punctuation, no more than 12 characters. This is usually a form of your company or organization name.
Examples: BofAmgrs, HarvardLaw, CalDeptFin, Acme03
Administrator Password Please do not choose something obvious. Avoid variations on your account name or "PulseCheck." This password is used to access and modify your account. Do not give this password out to others.
User Password (created later) You will announce this password to those you want to have take the survey. This one should be easy for the survey population to remember. You will create this password when you create your Survey.

We need your contact information for billing and to provide the status of your survey. We do not give this information to anyone else (we feel the same way you do about unsolicited calls, mail, and spam ).

- For Name, please provide first and last. Example:  Jane Doe

- For Phone, please enter digits only. Example:  916   446   4958

- For Address, just ignore unused lines

- For State, please use 2-letter abbreviation. Examples:  CA, VA

-Use either Zip or Zip+4: Examples: 23692,  23692-1762
